Social Security System – SS Number Application Requirements, Branches, Procedures

This post is all about the application for an SS Number from SSS (Social Security System).

The other day I was at the Caloocan office of SSS to get an SS number. It took me one hour. It should not have taken so long if it wasn’t because of the queue.

PLEASE NOTE: During my application for an SS number, I had to submit photocopies of my ID’s and present the original copies. You might want to photocopy your ID’s, just to avoid inconvenience.

What are the requirements for SSS Number Application?

• Form E-1, which can be acquired from the SSS office or downloaded from the following resources:
  • Download SSS Form E-1 from Oks.ph
  • Download SSS Form E-1 from Docstoc
  • Download SSS Form E-1 from the official SSS website
    
• Valid document(s) for identification (Scroll down and see the following lists)

Valid documents for Identification, as written at the back of the Form E-1 that I acquired from SSS Caloocan

• Certified True Copy of one of the following:
  • Birth Certificate
  • Baptismal Certificate (in absence of birth certificate)
  • Passport

• In the absence of any of the above, any two of the following should be submitted:
  Record of Employment (Employer ID) 
  • GSIS Member’s Record
  • Certificate from National Archive
  • Alien Certificate of Registration
  • Birth / Baptismal Certificate of Children
  • Marriage Contract
  • School / Voter’s ID Card
  • Driver’s License
  • Joint Affidavit

Valid Documents for Identification, from official SSS government website

Original/certified true copy and photocopy of any of the following:

• baptismal certificate,
• birth certificate,
• driver’s license,
• passport,
• Professional Regulation Commission (PRC) Card, or
• Seaman’s Book

In the absence of the above documents, any two of the following, one of which recent photo and date of birth, are acceptable.

• Alien Certificate of Registration
• ATM card with the cardholder’s name
• ATM card and certification from the issuing bank that the account number belongs to the cardholder, if the card does not bear the cardholder’s name.
• Bank Account Passbook
• Birth/Baptismal Certificate of children
• Certificate form:
• -Office of the Southern/Northern Cultural Communities
• -Office of Muslim Affairs
• Certificate of Naturalization issued by the Bureau of Immigration
• Company ID
• Company Representative Authorization Card issued by SSS
• Credit Card
• Fishermen’s Card issued by Bureau of Fisheries & Aquatic Resources (BFAR)
• GSIS Card
• GSIS Certificate of Membership
• Health or Medical Card
• ID Card issued by Local Government Units (e.g. Barangay/ Municipality/City)
• ID Card issued by professional associations recognized by PRC
• Life Insurance Policy
• Marriage Contract
• NBI Clearance
• Overseas Workers Welfare Administration (OWWA) Card
• Pag-Ibig Member’s Data Form
• Permit to Carry Firearms issued by the Firearms & Explosives Unit of the Philippine National Police (PNP)
• PHIC Member’s Data Record
• Police Clearance
• Postal ID Card
• School ID
• Senior Citizen Card
• Tax Identification Card/Affidavit
• Transcript of School Records
• Voter’s Identification Card/Affidavit

Any other information written at the back of the Form E-1?

Other statements written at the back of the Form E-1:
An SS Number may be issued with or without proper identification.

However, to facilitate availment of benefits, it is suggested that any of the following documents should be submitted upon registration or shortly thereafter.

The assignment of SS number does not automatically make the registrant nor his dependents SSS members. To become an SSS member, one must first be employed by a covered employer.

An SS number is a lifetime number. No one should have more than one SS number.

What are the procedures in applying for an SS number?

What I did at the Caloocan branch were… prepared my ID’s, filled up a Form E-1, got in the loooooong queue, submitted my documents, put a thumb mark on my form, and was handed a copy of Form E-1 with my SS number.

Is there any cost in getting an SS Number

Fortunately, no. It’s free. 🙂
